All volunteers participating in Dallastown Cougar youth activities must create an account on our website.  Once you create your account, you must upload the following documents: current photo ID, State Background check certificate, Proof of fingerprinting (If you have been a resident of PA for less than 10 years,) Child Abuse History Certificate and signed Mandated Reporter Form.   You will also need to accept our Coach/Team Parent Code of Conduct waiver and the YCYFA Volunteer Code of Conduct waiver.  If you have been a resident of PA for longer than 10 years, you must accept the YCYFA FBI waiver.  Anyone applying for a Coach position must complete the CDC Heads Up Concussion course and upload their certificate.  All clearance websites and forms are located under the Volunteer tab on our website.  By creating an account, it doesn't commit you to volunteering or guarantee you a coaching position.  It does give us a list of individuals and their contact information to begin the selection process.

Required Annually:

• Copy of PA Driver's License or ID Card.
• All head and assistant coaches CDC Heads Up Concussion Certificate: headsup.cdc.gov
